Personally, I love the Mass Effect Trilogy and it’s the kind of game you naturally tend to replay anyways for hundreds of hours, so it’s the ONE series where I actually liked the idea of giving Insanity a shot. It was wild, but I beat it and have no regrets.
These modes by and large may be more difficult overall, but the hardest boss I've ever faced in any game still will come down to a handful of Mythic Raid bosses in the first few weeks after they're released. I know the internet likes to call everyone a noob for admitting something like that, but I'd rather fight damn near any super boss in any franchise than limp through Kil'jaedan Mythic the first month after ToS is released.

Beat Doom on Nightmare. Took me freaking DAYS... and two destroyed keyboards. The advice is correct; know where EVERYTHING is, where enemies spawn, and run like the Cyberdemon is eternally chasing you.
Call of Duty World at War was a truly hellish challenge on veteran, but it took me 2 weeks with a mission or 2, playing 3-4 hours/day xD
But it was very fun and i sorta leveled up on fps games so, if you like a game, play it in all ways, that's my opinion :)

I beat COD MW, WAW, MW2, BO, and MW3 all on Veteran. And yes, WAW was freakin hard, especially the level with the Japanese Shrines at the end of the game. There's smoke everywhere and you cant see anything.
you think nightmare in the original doom is hard? apparently someone hasn't tried "mein leben!" (german for "my life!") mode in wolfenstein 2: the new colossus. but to unlock it (because it's locked by default) you have to beat the game in "i am death incarnate!" mode. similar to "i am death incarnate!" but you only have one life, with no autosaves and manual saves are disabled, similar to the secret "ironman" difficulty from wolfenstein: the new order.
I know this is PS but I have to say this: Halo 2 on legendary is the stupidest thing I've ever tried. A friend and myself were trying co-op and we thought we were pretty good at Halo 2 but we got our asses handed to us. The saddest part...we never made it past the first enemy encounter. We eventually just gave up.
